Rehabilitation Management Plan Template
A Rehabilitation Management Plan is a comprehensive plan for coordinating and managing the treatment, rehabilitation and return to work of an injured and/or ill nurse.
The RTW Coordinator is responsible for developing the Rehabilitation Management Plan in consultation with the injured nurse, NUM/ANUM, treating medical practitioners and any other relevant party.
Please note this plan cannot replace the Offer of Suitable Employment and Return to Work Plan, as an employer has a legislative obligation to undertake these.
A Rehabilitation Management Plan includes:
- contact details for all parties
- demonstration of consultation with the injured nurse, NUM/ANUM, treating medical practitioner in the development of Rehabilitation Management Plan
- signed agreement by the injured nurse, NUM/ANUM, treating medical practitioner and RTW Coordinator, to cooperate and comply with the Rehabilitation Management Plan
- outline of communication pathways
- assessment of injury/illness
- medical and allied health management
- offer of suitable duties
- identification of retraining where applicable
- occupational rehabilitation services
- roles and obligations of all parties
- scheduled dates for review.
